Any Passenger/Individual/Basecamp Helicopter staff identifying any hazard or any activities that may affect the safety of people, damage of equipment or aircraft/ vehicles should fill up this form or can also report in written format and drop into nearby safety reporting boxes. The name of reporter shall remain confidential; however, it is optional for a reporter to disclose his/her identity.

Note: This submitted report is non-punitive and no action shall be taken against any employee who discloses the safety concern unless such disclosures indicate beyond any reasonable doubt, gross negligence or a deliberate or willful or intentional disregard of regulations or procedures.

सम्बन्धित कुनै व्यक्ति वेसक्याम्प हेलिकपटर कर्मचारीले, जहाज, बिमानस्थल सवारी साधन तथा अन्य कुनै उपकरणहरुको सुरक्षासंग सम्बन्धित जोखिम तथा सम्भावित जोखिमको बारेमा कसैले पनि प्रत्यक्ष देखेमा वा सुचना पाएमा, त्यस्ता जोखिमको बारेमा सम्पुर्ण विवरण उल्लेख गरि यो फारम भर्नका लागि सबैलाई प्रोत्साहन गरिन्छ । यस फारममा उल्लेख गरिएको जानकारी केवल विमानसेवाको हवाई सुरक्षालाई अझ सुदृण गर्न मात्र प्रयोग हुनेछ । त्यसैकारण रिपोर्ट गर्नेको नाम र पद उल्लेख गर्न जरुरी छैन अथवा त्यस्ता जानकारी गोप्य राखिनेछ साथै रिपोर्ट गर्ने ब्यक्ति वा कुनै पनि कर्मचारीलाई सजाय दिने तथा नकरात्मक रुपमा लिइने कार्य हुने छैन ।
